                      An allied health professional known as a medical scribe concentrates on recording doctor-patient interactions as they happen, as during physical examinations. The role of the medical scribe is to facilitate the effective and efficient performance of doctors. The Scribe's primary responsibility is to write down the patient's story so that the doctor can concentrate on providing the patient with high-quality care. The medical scribe will document the main complaint, the history of the current illness, the evaluation of the systems, prior medical history, family history, social history, medications, allergies, physical exam, assessment, and plan. Any orders and labs that the doctor places will also be recorded by the Medical Scribe.

A medical scribe must undergo training before they may begin working. Medical scribes are in charge of accurately and completely recording patient information as it emerges during a visit to the doctor. The information acquired by the scribe is then used to create a complete report for the patient's medical file.
