                   Medical scribes are employed by doctors and other healthcare professionals to assist them with patient documentation. As the doctor speaks, the scribe's main responsibility is to accurately capture all relevant information and enter it into the patient's electronic medical record (EMR). This includes medical histories, physical exam findings, test results, diagnoses, suggested therapeutic regimens, and other applicable data. The scribe may also help with ordering tests, medications, and other types of therapy. Although medical scribes are not doctors, they are crucial to the patient's care since they ensure that the patient's medical record is complete and accurate.

The function of a medical scribe is to assist the doctor in keeping medical records. An electronic medical record (EMR) system must contain information on the patient's visit, including the patient's history, the findings of the physical examination, and any procedures or tests that were performed. Along with accepting calls and scheduling appointments, medical scribes can help with other clinical tasks like filing orders, collecting samples, and preparing lab tests. Another responsibility of medical scribes is to ensure that the medical records are accurate and up to date.
