MEDICAL SCRIBING 

                  A medical scribe is a person who records medical information from a patient's appointment with a physician. The medical scribe is in responsibility of precisely inputting all pertinent information, such as the patient's medical history, symptoms, diagnosis, and treatment plan, into the patient's medical chart. Scribes frequently work closely with the doctor from the time of the initial patient interview until the conclusion of the appointment.

The scribe's responsibility is to accurately document the doctor-patient conversations and the information placed into the patient's chart.
